Job Summary and Qualifications

1. Perform appropriate assessments to include psychological, physical, environmental and functional needs.

2. Provide patient and family education specific to the patient’s disease process.

3. Serve as a resource for education for patient, family, staff, hospital and community.

4. Communicate with all members of the transplant team to include hospital staff and referring physician/staff

5. Participate in the collection/entry of data in the patient medical record as required by all regulatory agencies

6. Document assessments, interventions and follow-up accurately and timely

7. Coordinate care through the phases of transplantation with the transplant team.

What qualifications you will need:

• 1. Mandatory: Graduate of an accredited registered nursing program.

Methodist Hospital Specialty and Transplant has been nationally recognized for its record setting kidney, liver and pancreas transplant programs. It is home to the Texas Transplant Institute and is the largest living donor kidney transplant program in the US.
